Health Questionnaire

To attend an online class or a face to face class we need you to complete a Student Health Questionnaire. The easiest way to do this is online.  Enter all of your details and then click submit and the form is sent to us.  Access our online questionnaire here: Yogasmiths Student Health Questionnaire

The online Yogasmiths Student Health Questionnaire can also be used to update the information we hold about you. Have you moved house? Do you have a new phone number or emergency contact? Any new health issues that we need to be aware of?

If attending a face to face class, we would advise new students to arrive at least 5 minutes before the start of class to complete a short health questionnaire. Alternatively you could always bring this with you to class completed. You can find the questionnaire via these links, a Word doc and a PDF version;

Yoga isn’t easy. It is tough discipline but rewarding! And the benefits of yoga aren’t instantaneous but the result of continued practice. Yoga gives us the space to take the awareness inwards bringing the body, mind and spirit into harmony.”

His book is not only aesthetically very pleasing but it is full of useful tips and represents a great practical guide.
